Step 1
Begin a new Word document by creating an APA-formatted title page. Then create the first page of your windshield survey assignment by inserting the heading, “Introduction to the Community.”
Identify the community you will use with the city and state and provide a one-paragraph description of the community. Your community should be the area where you live or the area surrounding your work setting. The community must include a residential area and be broad enough to answer the survey questions. You do not need to include demographic data.

What did you observe about this population?
Are there common genetic factors related to increased risk factors for various disease processes (i.e., diabetes or hypertension)?
Describe a theoretical framework from which to address health concerns/risk factors.
Step 4
Begin a new section in your Word document with the heading “Conclusion.” Describe 3-5 potential community health problems for the identified vulnerable population.
Step 5
Finalize and proofread your paper. It should contain APA-formatted in-text citations and have an APA-formatted reference page. Your paper should be 3-4 pages, not counting the title and reference pages.
